Our website includes a location finder tool that allows you to search by ZIP code to find a Legacy Hospice branch near you. The ZIP code you enter is used solely to display relevant location results and is not stored or linked to your personal information.

We do not use Information collected through our Site to make decisions about your medical care or eligibility for hospice services. Any clinical assessments or care decisions are made through our formal intake and clinical evaluation processes.

If you submit a contact form on our website, a member of our team may follow up with you by phone or email based on the Information you provide. These communications are related to the inquiry you submitted and are intended to help connect you with the care or information you need.

You may have the right not to be discriminated against for exercising any of your rights under applicable law.  Please note, however, that we may be permitted to offer certain benefits in connection with your voluntary participation in certain incentives and loyalty, rewards, or similar programs.
  • Withdrawal of Consent: Where we rely on consent to process your Personal Information, you may have the right to withdraw this consent. Please be aware that withdrawals of consent are not retroactive and do not impact our processing of your Personal Information prior to the withdrawal. Withdrawing consent also does not impact our processing of Personal Information conducted on other lawful grounds other than your consent.

Legacy may need to collect information from you to verify your identity, such as your email address or account information, before providing a substantive response to the request.  In accordance with applicable laws, you may designate an authorized agent to make requests on your behalf to exercise your rights.  Before accepting such a request from an agent, we will require that the agent provide proof you have authorized them to act on your behalf, and we may need you to verify your identity directly with us.  We will respond to your request in a timely manner as required under applicable law.

Children’s Privacy

Our website is not directed at children under the age of 13, and we do not knowingly collect personal information from children. ).  In accordance with the Children’s Online Privacy Protection Act (“COPPA”), we will never knowingly request or solicit Personal Information from anyone under the age of thirteen (13) without verifiable parental consent.  In the event that we receive actual knowledge that we have collected such Personal Information without the requisite and verifiable parental consent, we will delete that information from our database as quickly as is practical.  Legacy reserves the right to request proof of age at any stage so that we can verify that minors are not using the Service(s).
