Medical
Vision
Driving to work, reading a news article and watching TV are all activities you likely perform every day. Your ability to do all these activities, though, depends on your vision and eye health. Vision insurance can help you maintain your vision as well as detect various health problems.
Dental
In addition to protecting your smile, dental insurance helps pay for dental care and usually includes regular checkups, cleanings and X-rays. Several studies suggest that oral diseases, such as periodontitis (gum disease), can affect other areas of your body—including your heart. Receiving regular dental care can protect you and your family from the high cost of dental disease and surgery.
Life
Legacy Hospice provides full-time employees with Basic Life and Accidental Death and Dismemberment (AD&D) Insurance in the amount of 1 times your annual salary to a maximum $150,000 and pays the full cost of this benefit.
HSA
Health Saving Accounts: Health Savings Accounts (HSAs) are a great way to save money and budget for qualified medical expenses. HSAs are tax-advantaged savings accounts that accompany high deductible health plans (HDHPs). HDHPs offer lower monthly premiums in exchange for a higher deductible (the amount you pay before insurance kicks in).
FSA
Flexible Spending Account: A Flexible Spending Account (FSA) is a special account you put money into that you use to pay for qualifying out-of-pocket health care costs. You do not pay taxes on this money. This means you will save an amount equal to the taxes you would have paid on the money you set aside.
DFSA
This type of account is designed to assist with expenses related to qualified daycare or elder care expenses for qualified dependents (children younger than 13 years old, and adult dependents incapable of caring for themselves).
Disability
Voluntary Life AD & D
Legacy Hospice pays for basic life insurance; however, some employees may want to purchase additional coverage. Think about your personal circumstances.
Supplemental Assurance Plans
Critical Illness Plan: The plan provides you with a lump sum benefit upon diagnosis of any qualified critical illness. Benefits are paid directly to you and can be used at your discretion for things like childcare, transportation, copays or deductibles. It also provides a wellness benefit when you complete a wellness screening.
Cancer & Hospital Indemnity: Employees will be offered a choice of $15,000 or $30,000 Guaranteed Issue Coverage. Spouse / Domestic partner will be offered 50% and Child (ren) will be offered 50% of employee benefit amount. Actual benefit payout is dependent on full of partial diagnosis.
PTO
At Legacy, PDO is a gratuity to employees and not earned by the employee.
• Paid Days Off (PDO) will be available as a gratuity to a new employee after completing a satisfactory probationary period.
• The new employees will be credited with PDO time beginning with the pay period in which they were hired.
• PDO are awarded as gratuity by year(s) of service with Legacy Hospice. The schedule for PDO is as follows:
